We are simplifying the process to apply for bankruptcy, combining the Statement of Affairs and Debtor's Petition forms and moving this service online.

We are simplifying the process to apply for bankruptcy and moving this service online on 2 January 2020.
As part of this work we are improving the Consent to Act form and would like to hear your feedback.
For more information on what’s changing see: Bankruptcy Online
The Consent to Act form can be used by people applying for bankruptcy and would prefer a particular registered trustee to manage their bankruptcy.
Registered trustees can also use this form when they want to assign a bankrupt estate or personal insolvency agreement to themselves.
We are seeking your feedback on the new form. View the original and updated Consent to Act form here:
Feedback closed on 30 October 2019. Thank you to everyone who provided responses.
